TNC Community is recognized as a community resource for helping people improve the quality of their lives. We work to achieve this by:
- Providing support to the individuals we serve in developing personal goals and working to achieve these goals.
- Promoting the development of interdependent community partnerships to allow integration into the community.
Selecting a home for your loved one is a difficult process. There are many topics to discuss as you visit each facility.

Supported Living

TNC Community offers multiple six-bedroom group homes and 2 two-bedroom Individual Supported Living locations, all with 24 hour supervision. There is always someone awake to make sure the consumers are cared for. Assistance is provided for daily living skills including meal time, grooming, hygiene, etc. and transporation is available for most outings.

- Will the consumer have input on what he or she eats?
- Will the staff help with medication, personal hygiene, meal times, etc.?
- If the consumer wants to attend a community event, will transportation be provided?
- Can the consumer or the family make choices for the consumer?

Community Integration
|

TNC Community provides assistance with planning activities in the community with transportation available for these outings.
|

- Will staff assist with vacation plans for the consumer?
- Will the consumer be able to continue attending the church of his or her choice?

Residential Habilitation
|
| |
TNC Staff work with the residents to achieve personalized goals and to accomplish as much independence as desired.

- Can I maintain my specific PCP plans?
- Will the staff help a resident achieve personal goals?

Healthcare Oversight/Management
|
| |
TNC has a Community Registered Nurse on staff and provides a monthly health status review. In addition, TNC offers 24 hour on-call healthcare services including pharmaceutical, assistance with scheduling doctor's appointments, transportation to the appointments and a weekly review of medications and medical status by a Health Support Coordinator. In the event that hospitalization is needed, a staff member will be with the consumer at the hospital until the family arrives.

- Can the consumer keep his or her current doctor?
- How often will the consumer see the doctor?
- Can the consumer or family member make doctor's appointments?
- What happens if the consumer has an emergency during the night?
- If a consumer needs to be transported to the hospital, will a staff member be with the consumer?
- What if a resident runs out of medication?
- What happens if a consumer isn't feeling well

Nutritional Guidance

TNC has contracted with a registered dietician for nutritional evaluations and education. Assistance is provided for specialized menu planning.
